YOU AND EACH OF YOU, ARE HEREBY NOTIFIED, the State of Oklahoma through Matthew J. Ballard, District Attorney in and for the Twelfth Judicial District for the State of Oklahoma, has filed a PETITION stating with particularity, the facts, which being the children under and within the purview of the statutes of the State of Oklahoma relating to the children, and the relief requested, specifically praying for the ADJUDICATION HEARING of the case involving the above named child and that control and custody of said juvenile be vested in such person, state agency or institution as the Court may deem to be in the best interest of said juvenile. Information of child, K.A.W., dob: 03/21/1998 born at Claremore, Oklahoma. The father of the above named child is John Williams, and the mother is Laticia Nicole Williams. FURTHER, that the above named parents, guardians and any interested persons must appear before the District Court in and for Mayes County, Juvenile Division thereof, in the Courtroom at Pryor, Mayes County, Oklahoma, at the hour of 9:30 a.m. on the 23rd day of February, 2016, for ADJUCIATION HEARING when and where the Court will proceed to hear the merits of said application and grant such relief or make such orders as the Court deems necessary and proper in these premises.
